After scheduling your Initial Evaluation:

1. Please download and fill out the New Patient Forms (See Below)

2. Please arrive 15 minutes early

3. Please have your driver's license and insurance cards ready.

4. Please wear loose clothing

5. The 1st visit, and subsequent visits will last about 45 minutes to an hour. A typical Physical Therapy intervention may last 3 to 6 weeks, based on your goals. Please bring a calendar to schedule your appointments. 

6. If you have any questions regarding Physical Therapy, any one of our clinicians would be happy to talk with you. Please arrange this with our Customer Service Associate.
